Important Notes for 2025 Diplomate Candidates:

• Database:
  You do NOT need to submit any of your required credentialing information through the new ACVNU database. The 2023-2025 class of residents will continue to use the paper forms and the provided excel spreadsheet.

• Retroactive Credit:
  If you received full retroactive credit for your case logs or clinical proficiencies, you do NOT need to resubmit those logs.
  If you received partial or no retroactive credit, you must submit your full, completed case clinical proficiency logs.

• Submission Timing:
  Please do NOT submit your credentialing packet until all required items are complete and available for review, including your publication.

• Credentialing Review:
  You may submit your packet at any time once complete.
  For this first round of reviews, if you submit by November 30, you will receive a decision no later than January 9, 2026.
  (This window will be adjusted if Certifying Examination results are not yet available by that time.)
